Service Description

There will be a $15 administrative fee. An authorization of release of records will need to be signed and a copy of your valid picture ID will need to be attached prior to releasing information. Please Allow 1 business day to process documents.

Cancellation Policy

We ask you reschedule or cancel your appointment at least 24 hours prior. There will be a $15 rescheduling fee due before any services are rendered.
